Letrozole is an oral non-steroidal aromatase inhibitor prescribed for the treatment of local or metastatic breast cancer - either hormone receptor positive, or with an unknown receptor status, in postmenopausal women.
Letrozole is used to treat early breast cancer in postmenopausal women who have received 5 years of adjuvant tamoxifen therapy; also to treat advanced breast cancer in women with disease progression after anti-estrogen therapy. It can also be used to treat infertility due to the polycystic ovary syndrome (PCOS).
Letrozole is known to delay the fusing of the growth plates in adolescents (what is likely to boost the effectiveness of growth hormone) therefore, letrozole can also be used to treat children with short stature.
Letrozole reversibly inhibits the aromatase enzyme, which normally converts androgens (like testosterone) to estrogens. Letrozole binds the heme on P450 unit of the aromatase enzyme making it unable to carry out its usual functions this way blocking production of the estrogens. This binding action is specific: letrozole selectively inhibits gonadal steroidogenesis but has no significant effect on adrenal mineralocorticoid or glucocorticoid synthesis.
Since breast tissue is stimulated by estrogens, decreasing their production suppresses recurrence of the breast tumour tissue. In adult tumour-bearing females, letrozole is as effective as ovariectomy in reducing uterine weight, increasing serum LH, and causing the regression of estrogen-dependent tumours. Unlike ovariectomy, letrozole does not increase serum FSH levels.
Doctor’s instructions should be followed at all times. Letrozole should be not taken:
Doctor or pharmacist should be consulted if the patient has a severe kidney or liver disease, has a history of osteoporosis or bone fractures
Day 1: 0.50mg Letrozole
Day 2: 1.0mg Letrozole
Day 3: 1.5mg Letrozole
Day 4: 2.0mg Letrozole
Day 5: 2.5mg Letrozole
Use until gyno subsides then:
Day 1: 2.0mg Letrozole
Day 2: 1.5mg Letrozole
Day 3: 1.0mg Letrozole
Day 4: 0.50mg Letrozole
Day 5: 0.25mg Letrozole
Very important: Then take 20mg Tamoxifen (Nolvadex) per day for 2 weeks
Take 2.5mg of letrozole from day 2 to day 7 of period week only. Femara is only used 5 days per month for this purpose.
Side effects are usually mild and include signs of hypoestrogenism: headache, joint pain, fatigue, hair thinning, in women: vaginal dryness and vaginal bleeding. The mmost common side effects include hot flushes, increased level of cholesterol (hypercholesterolaemia), fatigue, increased sweating, and pain in bones and joints (arthralgia). Long term use may lead to osteoporosis, thus treatment with letrozole can be combined with anti-osteoporosis medication.
Letrozole can reduce estrogen levels by 98 percent while increasing testosterone levels at the same time. This can be utilised for use during steroid cycles: to reduce bloating due to excess water retention and to prevent formation of gynecomastia (side effect of some anabolic steroids). Daily dose above 2.5mg is known to potentially temporarily kill sex drive. Use of doses over 5mg per day for extended periods may cause kidney problems.
